The Neurology Department at Caen Normandy University Hospital comprises four units: the Neurovascular Unit, the General Neurology Unit, the Day Hospitalization Unit, and Neurological Functional Explorations, as well as an outpatient clinic. It admits 5,500 inpatients and provides over 6,500 outpatient consultations annually.

    Activities

    Presentation

    The department treats all neurological conditions in adults, whether acute or chronic. It is the reference and referral center for other neurology departments in the western Normandy region (Alençon, Avranches, Cherbourg, Lisieux, Saint-Lô).

    A 24/7 on-site team ensures continuous care for all urgent neurological conditions, particularly strokes , potentially covering the entire region through telemedicine. This work is carried out in close collaboration with the neuroimaging technical platform (CT scan, MRI, interventional neuroradiology) , neurological functional testing services (electroneuromyography, electroencephalography, evoked potentials, MRT) , and the functional rehabilitation department.

    The department serves as a teaching and university training ground for medical students (externs and international students), interns, and junior doctors. It is an integral part of and collaborates closely with two INSERM : U1077 "Neuropsychology and Imaging of Human Memory"; and U1237 "Pathophysiology and Imaging of Neurological Disorders."

    Consultations

    Consultations are available from Monday morning to Friday afternoon. They are provided by all the neurologists in the neurology department. These include general neurology consultations as well as specialized consultations dedicated to specific pathologies, drawing on the expertise of reference centers. Some consultations are held at other locations: Alençon, Avranches, and Flers.

    Neurovascular Unit

    The Neurovascular Unit ( NVU ) has a capacity of 26 beds, including 10 intensive care beds. It admits patients with a stroke (cerebral infarction, transient ischemic attack, cerebral hemorrhage, cerebral venous thrombosis) and has the staff and equipment necessary to provide acute stroke care with continuous monitoring and privileged access to neuroimaging technology. In emergency situations, revascularization techniques can be performed for patients with a cerebral infarction, either via infusion (thrombolysis) and/or mechanical means (thrombectomy), for which Caen Normandy University Hospital is the regional referral center and beyond. All paramedical staff (nurses, nursing assistants, speech therapists, physiotherapists, occupational therapists, neuropsychologists) are also trained in the treatment of stroke patients .

    Multidisciplinary post- stroke consultations with a nurse specially trained in neurovascular pathology are organized following hospitalizations at the Stroke Unit. Similarly, consultations with rehabilitation specialists are available to assess post- stroke sequelae and adjust treatment accordingly.

    This unit is also an expert center for rare neurovascular diseases (CADASIL, hereditary cerebral amyloid angiopathy, etc.).

    General Neurology Unit

    TheGeneral Neurology Unit (GNU) has a capacity of 20 beds. It admits patients with neurological symptoms requiring diagnostic evaluation, further investigations or the implementation and/or adaptation of treatment (e.g., epileptic seizures, headaches, vertigo, abnormal movements, multiple sclerosis flare-ups, brain tumors).

    It also admits and treats acute complications of chronic neurological diseases such as dementia, Parkinson's disease, and amyotrophic lateral sclerosis. It also includes two palliative care beds dedicated to neurological conditions.

    Day Hospitalization Unit and Week Hospitalization Unit

    TheDay Hospitalization Unit (DHU) has a capacity of 4 beds and 4 armchairs and theWeek Hospitalization Unit (WHU) has a capacity of 4 beds.

    They accommodate patients requiring diagnostic assessments including additional programmed investigations (lumbar puncture, arteriography, MRI…), treatment courses in a hospital setting (multiple sclerosis, chronic polyradiculoneuritis…), or a follow-up assessment of chronic neurological diseases (amyotrophic lateral sclerosis, neuromuscular pathologies, Parkinson’s disease…).

    Neurological functional explorations

    The Neurological Functional Explorations department welcomes patients (children and adults) for electroencephalogram (EEG), electromyogram (EMG), evoked potentials and repetitive transcranial magnetic stimulation (rTMS).

    Specialized consultations for epilepsy are also offered.

    The neurological functional explorations department has a significant cross-departmental activity within the hospital:

    • Performing EEGs for all hospital departments, particularly in neurology, medical and surgical intensive care, pediatrics, neonatology, geriatrics, emergency services, etc.
    • Intraoperative neurophysiological monitoring in pediatric orthopedic surgery and neurosurgery (interventions on scoliosis, brain tumors, etc.).

    The department works closely with the Neurology department for the care of epileptic patients. Consultations are provided by two neurologists from the Neurological Functional Explorations department, as well as by practitioners from the neurology department.

    For epilepsy consultations, appointments are offered upon written request from the patient's general practitioner and/or specialist. Neurophysiological examinations, essential for monitoring this condition, are performed in the Neurological Functional Explorations Department for routine electroencephalograms (EEGs), and in theSleep Unit for EEGs . These examinations are conducted by paramedical staff specifically trained in this condition.

    A specific Epilepsy-Work consultation is offered in collaboration with Professor Bénédicte Clin-Godard from the Occupational Health and Occupational Pathology department .


    A number of electroencephalograms performed in other hospitals in the region, and in particular the majority of EEGs , are sent to us for remote interpretation via a dedicated platform.

    Furthermore, more confidential activities because they are highly specialized are carried out only at the University Hospital of Caen (single fiber, oculography in parkinsonian syndromes +, rTMS in functional paralysis, EMG of the perineum,...).

    Reference centers and expertise

    Consultations

    Consultations are provided by all the neurologists in the department; some are conducted at external locations: Alençon, Avranches, Flers. These include general neurology consultations as well as others that are specific and dedicated to particular pathologies.

    Six "reference centers" and "competence centers" structure these consultations and the resulting care:

    1. Memory Resource and Research Centre (CMRR) for all pathologies involving cognitive disorders
    2. Rare or Early-Onset Dementia Competence Centre for frontotemporal lobar degenerations, primary progressive aphasias, corticobasal degenerations, and progressive supranuclear palsies
    3. Multiple Sclerosis Resource and Competence Centre (CRC-SEP) for multiple sclerosis and related inflammatory diseases
    4. Amyotrophic Lateral Sclerosis Resource and Competence Center (CRC-ALS) for ALS and motor neuron diseases
    5. Parkinson's Expert Center (CEP) for Parkinson's disease and parkinsonian syndromes
    6. Rare Vascular Diseases of the Brain and Eye (CERVCO)
    7. Neuromuscular Disease Competence Center
    8. Neurogenetic Disease Competence Center (NEUROGENE)

    Specialized consultations for epilepsy and sleep disorders are also organised by and in collaboration with the neurological functional explorations service and by and in collaboration with the functional rehabilitation service for neuromuscular pathologies (competence centre).

    Neurological functional explorations

    What is an electroencephalogram?

    Electroencephalography is a method of brain exploration that measures the electrical activity of neurons, and its variations over time, by means of electrodes placed on the scalp.

    It allows the study of brain function in healthy people (for example, brain maturation in babies, or the different stages of sleep), but also the diagnosis of certain diseases that modify brain electrical activity, in particular epilepsy and disorders of vigilance or comas.

    It is a completely safe and painless "non-invasive" technique, most often requiring no preparation other than clean, dry hair, and can be performed in all circumstances, on an outpatient basis, in intensive care, or at the patient's bedside, in both children and adults. Prior sleep deprivation is sometimes required.

    For outpatient examinations, except in the case of very young children or patients with intellectual disabilities, a minimum level of patient cooperation is desirable for a quality examination (remaining calm, opening and closing eyes, blowing, etc.).

    The examination generally lasts less than 30 minutes, but longer recordings are possible, for example during sleep (a nap or overnight) or over several consecutive days to investigate certain fainting spells and epilepsies. These latter examinations are carried out in the Sleep Unit.

    Neurological functional explorations

    What is an electroneuromyogram (ENMG)?

    This involves an examination of the nerves and muscles performed by a neurologist specializing in this technique:

    The exam is conducted in two parts:

    • The first part, called nerve stimulation, studies nerve function. An electrical stimulator placed on the skin near the nerve activates the nerve, triggering a muscle contraction or a sensory potential. This allows the nerve conduction velocity to be measured and any loss of nerve fibers or damage to the myelin sheath surrounding them. This can confirm the diagnosis of peripheral nerve disease (peripheral neuropathy) or motor neuron disease (the motor neuron is a cell located in the spinal cord that gives rise to the motor nerve). Repetitive stimulation can also reveal a disorder of transmission between nerve and muscle (as in myasthenia gravis).
    • A second part, the electromyogram, is carried out using a needle electrode that is inserted into the muscle: this allows us to study the activity of the muscle fibers at rest and then during exertion, in order to study the type of recruitment of muscle fibers during exertion, which allows us to direct towards a specific pathology of the muscle fibers (we then speak of myopathy) or towards a disease of the peripheral nerves or motor neurons.

    What is an evoked potential test?

    Evoked potentials are changes in brain electrical activity that can be recorded by electroencephalography.

    In clinical practice, these modifications are controlled and induced, depending on the sensory modality being assessed. Regardless of the modality being explored, the technique requires recording a consecutive series of responses, the average of which will exhibit characteristics of amplitude (size) and latency (time of appearance) that can be compared between normal and pathological situations.

    The modalities commonly explored in the department are evoked, somatosensory, and motor potentials. These examinations are of major importance in the investigation of spinal and peripheral neurological pathologies and can have significant prognostic value in the management of comatose patients in intensive care. This technique is also used in the department for intraoperative monitoring during spinal surgery.
