Hospital social services

You, your family, or those close to you can request assistance from the hospital social services department. They will assess your situation with you (administrative, family, social, and professional). They can intervene in areas such as: home care, referrals to appropriate facilities, and access to rights.

He will also work with all the staff to make your stay in the hospital easier.

The duties of the social worker

"The mission of social and educational assistants is to help patients, residents, and families experiencing social difficulties to prevent or overcome these difficulties, to maintain or regain their autonomy, and possibly to facilitate their social and professional integration.".

Activities

Patients, as well as their families or friends, can request the intervention of the hospital social services department. This department will assess their situation (administrative, family, social, professional) in consultation with the medical and care team.

The social services department can intervene in the following areas: home support, referrals to appropriate facilities, access to rights (administrative procedures, emergency assistance, protection of vulnerable individuals, etc.). It will also work with all staff to facilitate their hospital stay.

What are PASS ?

The Permanent Access to Healthcare Services (PASS) are medical and social support units that should facilitate access for disadvantaged people not only to the hospital system but also to institutional networks of care, reception and social support.

A team of professionals, consisting of a social worker, a nurse and a doctor, is available to help and support patients.
