      Consultations

      The consultation activity, which is significant, with over 10,000 urological consultations per year for 10 surgeons, concerns:

      • oncology (30%),
      • benign prostatic hyperplasia (20%), for which we prefer medical management
      • neuro-urology and incontinence (10%),
      • lithiasis (15%),
      • genitourinary infection (10%),
      • andrology, both in the field of erectile dysfunction and in that of male infertility (ministerial approval obtained in 2000).

      Many diagnostic or therapeutic procedures : outpatient endoscopies, ultrasound-guided prostate biopsies, urodynamic assessments…

      A psychologist, attached to the practice, provides consultations for treatment, when necessary, in the field of sexology and erectile dysfunction.

      Hospitalizations

      The management of urological cancers is comprehensive, from diagnosis to medical or surgical treatment, including end-stage care. The department works closely with various stakeholders, primarily the Oncology team at the University Hospital.

      The urology department uses the surgical robot (Da Vinci X) with a dual console. This allows for minimally invasive surgery with great finesse and precision. This tool is primarily used for the treatment of urological tumors (prostate cancer, kidney cancer with the partial nephrectomy technique, bladder cancer).

      We are proud to offer our patients cutting-edge technology that allows us to perform prostate biopsies with millimeter precision by fusing MRI andultrasound images. We use elastic ultrasound-MRI with a state-of-the-art machine. Biopsies are performed transperineally or transrectally under local or general anesthesia.

      Prostate adenoma system, Greenlight® Laser vaporization reducing hospital stays and postoperative bleeding. Recently, the department has acquired a new gold standard technique for treating prostate adenoma (Holmium laser enucleation). 

      The management of neuro-urology and incontinence is a field whose activity, which can be performed on an outpatient basis, is constantly and rapidly increasing. The latest techniques are used, in particular sacral neuromodulation , which can produce spectacular results in cases of severe bladder instability and chronic neurogenic urinary retention.

      A regular multidisciplinary consultation meeting (Perineal Clinic) has been in place since 2019 to comply with the new HAS. Gastroenterologists, digestive surgeons, gynecologists, and a psychologist are present.

      The surgical management of renal failure, in collaboration with the Nephrology-Hemodialysis of the University Hospital but also with the various nephrology teams in the region, is an important activity.

      We use "heated Ametycine" within the department. This therapy allows us to treat certain types of bladder cancer that are resistant to conventional treatments.

      In order to offer rapid care within our territory and avoid patient travel, all team members consult at least once a week in a public hospital:

      Finally, we have a dedicated consultation service for assisted reproductive technology and erectile dysfunction, staffed by a urologist and with the invaluable support of a sexologist psychologist who consults within the urology team (Ms. Caroline Devisme). The department has recently acquired equipment that allows for the management of erectile dysfunction and penile deviations (Pierer's disease) while avoiding invasive treatment (surgery).

      Pathologies

      Urological oncology

      The department treats all urogenital cancers, including cancers of the kidney, upper urinary tract, bladder, prostate, testicles, retroperitoneal tumors, and adrenal glands. It provides diagnosis and surgical treatment, either laparoscopic or conventional, depending on the case.

      The cases are discussed in a multidisciplinary consultation meeting with the team from the François Baclesse Centre, where urological surgery is not covered but where chemotherapy, when necessary, is covered.

      Neuro-urology

      Neuro-urology is the management of all urinary and sexual symptoms related to a neurological disease (multiple sclerosis, paraplegia, etc.) or without a clearly recognized cause.

      Explorations (urodynamic assessment, endoscopies), treatments (drugs, sacral neuromodulation, botulinum toxin, or even major surgery to protect the urinary system) are a recognized activity of the department.

      Incontinence, genitourinary prolapse

      This approach is carried out from symptom to treatment, often on an outpatient basis.

      We treat all types of incontinence, male and female. We have at our disposal all diagnostic and treatment techniques, from minimally invasive treatments (slings) to more specialized ones (sacral neuromodulation, botulinum toxin, artificial sphincter).

      All cases are discussed before surgery in a Multidisciplinary Consultation Meeting (RCP).

      Surgery for Chronic Kidney Failure

      Through nephrologists, we provide kidney transplants (deceased donor or living related donor), organ harvesting surgery and peritoneal access surgery for dialysis.

      Prostate adenoma

      This common condition may require drug treatment or more aggressive treatment (endoscopic resection of the prostate, radiofrequency and soon laser which can then be performed on an outpatient basis).

      Prostate cancer

      Urologists are responsible for ensuring the early diagnosis of this common disease, which is detected by PSA testing and digital rectal examination and confirmed by prostate biopsies performed by the urologist on an outpatient basis.

      Depending on the potential severity of the disease, treatments can range from simple monitoring to surgery (laparoscopic or conventional). We are an expert center for the Ablatherm treatment technique. Radiotherapy, when offered (brachytherapy with seeds, conformal radiotherapy), is performed at the François Baclesse Center, as are certain chemotherapies.

      Kidney tumors

      Kidney tumors, whether benign or, more often, malignant, usually require surgical treatment, necessitating partial or total removal of the diseased kidney, either by laparoscopy or conventional surgery.

      In more severe stages, the use of modern drug treatments (anti-angiogenic) is ensured by the team at the François Baclesse Centre.

      Bladder tumors

      Most often revealed by red urine (hematuria), they are initially treated through natural means; fortunately, surgical removal of the bladder is less frequent.

      Urinary stones

      Most often revealed by renal colic-type pain, urological treatment may prove necessary, whether urgently or at a later date.

      The use of extracorporeal lithotripsy is becoming less and less frequent since we have at our disposal a laser and miniaturized equipment allowing us to retrieve and destroy stones, whether they are in the kidney or the ureter.

      For larger kidney stones, we use percutaneous surgery, entering the kidney through a small incision in the skin. Conventional open surgery is rarely necessary.

      Erectile dysfunction

      The urology department handles the investigation and treatment of erectile dysfunction, regardless of its origin.

      Treatments

      The department regularly participates in the evaluation of pharmacological treatments, particularly in the field of oncology, within the framework of international multicenter therapeutic trials.

      The urology department was recently selected (February 2020) to participate in a multicenter study aimed at testing a new treatment for bladder tumors (heated Ametycine). This treatment began within the department more than two and a half years ago, and the department has a good understanding of the technique.

      Sacral neuromodulation is another area of ​​research, particularly for micturition disorders of neurological origin, and has been the subject of several presentations at conferences in France and abroad.

      The benefits of PET scans (positron emission tomography), thanks to the presence of equipment at the Caen site, are being studied and have been presented at the European Urology Congress.

      The urology department has been selected to participate in two large international multicenter studies, including an early phase study in bladder cancer.

      Publication in regular international journals.

      Medical information (Medical Information Systems Program – PMSI) and evaluation are also a concern of the service.
