If you do not speak French, please come accompanied as much as possible by a person who can translate.

Upon receipt of this form, we will contact you by phone or email as soon as possible. You will be informed of the follow -up possibilities, depending on the level of risk assessed of your pregnancy, and in collaboration with the liberal professionals of your choice, in order to guarantee you coordinated care.
