Optimizing drug prescriptions for the elderly

The elderly population is more susceptible to adverse drug reactions. All of these undesirable effects, called “iatrogenics”, are the cause of 20% of emergency hospitalizations for people over 75 years old. In this context, OMéDIT* Normandie has launched Optimedoc: a drug optimization program, in conjunction with ARS ** Normandie. In 2022, 12 institutions, including Caen University Hospital, joined the project in order to optimize the care of these elderly polymedicated patients (taking more than 5 drugs).

Optimédoc*** is an interdisciplinary and multi-professional course aimed at people over 75 years of age. The general practitioner, the dispensing pharmacist as well as the hospital support system made up of a hospital pharmacist and a geriatrician work together to fully reassess the medications of patients with a significant iatrogenic risk. Patients benefit from a personalized pharmaceutical plan. The patient's new prescriptions are adapted in line with their previous treatment and transmitted to all city health professionals.

Bimedoc® pharmaceutical care software was chosen to support city-hospital coordination. This innovative software helps with the re-evaluation of medications, it facilitates the analysis of drug interactions and helps in the detection of potentially inappropriate or missing medications. Bimedoc® allows personalized monitoring of drug treatment by generating a summary for hospital and community professionals.
